Dear Valued Patient,

As you may know, CNMRI, PA permanently closed on September 30, 2025.

All medical record requests are now being managed by Morgan Records Management, LLC (the “Records Custodian”). The Records Custodian will continue to maintain your records in accordance with all applicable confidentiality, security standards, and legal requirements.

Your medical records will be retained for no fewer than seven (7) years from the date of your last service with CNMRI, after which they will be securely destroyed in accordance with state regulations.

• There may be a brief transition period between the practice closure and the time Morgan Records Management gains full access to patient charts.
